On the evening of July 9, 2026, the Lancaster community was left grieving after a heartbreaking two-vehicle crash in Fairfield County claimed the lives of Miranda M. Maughmer, 25, of Lancaster, and her 3-year-old child. The devastating collision occurred at approximately 7:33 p.m. on Bremen Road SE near Lake Road SE in Berne Township, prompting a large emergency response from multiple agencies.
According to the Ohio State Highway Patrol, Miranda M. Maughmer was driving a 2023 Kia Forte eastbound on Bremen Road when, for reasons still under investigation, her vehicle crossed the center line and collided head-on with a westbound 2015 GMC Acadia driven by Faith E. Irwin, 33, of Bremen. The impact caused catastrophic injuries, and Maughmer was rushed by Lancaster EMS to Fairfield Medical Center, where she later passed away.
Tragically, two young children were riding in the rear seat of the Kia. Both were properly secured in child safety seats. The 3-year-old was transported to Fairfield Medical Center but sadly succumbed to injuries sustained in the crash. An 8-month-old child survived with minor injuries and continues to recover.
Faith E. Irwin was also transported to Fairfield Medical Center with non-life-threatening injuries. Authorities reported that neither driver was wearing a safety belt at the time of the collision.
The Ohio State Highway Patrol was assisted by the Fairfield County Sheriff’s Office, local fire and EMS departments, the Fairfield County Engineer’s Office, the Fairfield County Coroner’s Office, and Estep’s Towing. Investigators continue working to determine the circumstances that led to this tragic crash. The heartbreaking loss of a young mother and her child has deeply impacted the Lancaster community, as loved ones mourn two lives gone far too soon.
